About our community access support
Community access supports help you take part in community, social and recreational activities that matter to you. A support worker can help you plan, travel to, take part in and return from activities, building your confidence and independence along the way.
Supports are delivered in line with your NDIS plan goals and your choice and control. We'll talk with you about the activities you want to do and how we can support you to do them.
What community access support can include
- Support to attend appointments and community activities
- Shopping, banking and everyday errands
- Joining classes, clubs, groups and community events
- Building social connections and community participation
- Developing confidence and independence in the community
- Working towards the goals in your NDIS plan

NDIS funding
This support generally relates to the NDIS Core Supports category Assistance with Social, Economic and Community Participation. What can be funded depends on your individual plan.
Evident Life Care is an unregistered NDIS provider. We support participants whose plans are self-managed or plan-managed. Check with your plan manager or support coordinator before you start.
Who we support
We support NDIS participants and people living with disability across Perth, including Wellard. We also welcome enquiries from families, carers, support coordinators and plan managers.
Common questions
Is Evident Life Care a registered NDIS provider?
No. Evident Life Care is an unregistered NDIS provider. We can support NDIS participants whose plans are self-managed or plan-managed. We cannot support participants whose plans are NDIA-managed (agency-managed).
Can I use my NDIS funding with Evident Life Care?
If your plan is self-managed or plan-managed, you can generally choose to use unregistered providers. Talk with your plan manager or support coordinator about which supports in your plan can be used for our services.
Which areas do you cover?
We support NDIS participants and people living with disability across Perth, Western Australia, including Wellard and surrounding suburbs.
