About our transport assistance support
Transport supports help you get to the places you need to go when you can't use public transport without substantial difficulty. A support worker can travel with you, support you on arrival and help you get home again.
Transport can be arranged on its own or alongside other supports, such as community access. We'll talk through how transport is funded in your NDIS plan before you start.
What transport assistance support can include
- Travel to medical and allied health appointments
- Travel to shopping, banking and errands
- Travel to work, study or day programs
- Travel to social, recreational and community activities
- Support on arrival and on the return trip
- Building confidence to travel more independently

NDIS funding
This support generally relates to the NDIS Core Supports category Transport. What can be funded depends on your individual plan.
Evident Life Care is an unregistered NDIS provider. We support participants whose plans are self-managed or plan-managed. Check with your plan manager or support coordinator before you start.
Who we support
We support NDIS participants and people living with disability across Perth, including Wellard. We also welcome enquiries from families, carers, support coordinators and plan managers.
Common questions
Is Evident Life Care a registered NDIS provider?
No. Evident Life Care is an unregistered NDIS provider. We can support NDIS participants whose plans are self-managed or plan-managed. We cannot support participants whose plans are NDIA-managed (agency-managed).
Can I use my NDIS funding with Evident Life Care?
If your plan is self-managed or plan-managed, you can generally choose to use unregistered providers. Talk with your plan manager or support coordinator about which supports in your plan can be used for our services.

We support NDIS participants and people living with disability across Perth, Western Australia, including Wellard and surrounding suburbs.
